"If in that location is an error in or omission of data on the security prescription for schedule II-V controlled substance, the pharmacist demand merely contact the prescriber, hash out with the prescriber the nature of the error or omission, and receive clarification of what was really intended or needed to be added." "The pharmacist should and so comprise the right or omitted information onto the security prescription along with documentation that the prescriber was contacted to rectify the mistake or omission." -"A Guide to California Community Pharmacy Police 9th edition"

Yep the constabulary says this, but i have never in practice changed fentanyl patch to dilaudid tablets just by annotating the original Rx, electronic or otherwise... (this would autumn nether 'error' and technically I could exercise this?)

I have however, changed 5/300 vicodin to five/325 norco due to insurance issues later speaking w prescriber.

I'd change str or dosage form, wouldn't change from one drug completely to another.

level 2

But "technically" a change from fentanyl to norco wouldn't be illegal, according to the written police(?)

I've never been faced with this and would be shocked if an MD/Exercise office admitted to writing such an error, but technically the law says it'southward ok (?)

Side note: my state doesn't clarify and every chemist has their own unique opinion nearly what can and cannot be inverse on a c-Ii prescription with consultation from the prescriber. Its so much fun.

And so you go off federal law

Name, name, name

level 1

Merely because y'all tin can doesn't mean you should or are legally obligated to do then.

Federally, it's proper noun, name, and name. With consult yous can change anything but the patient's name, dosage form (drug proper name), and information technology has to be signed. Dosage form can get a niggling grayness with things like an fault in morphine 15mg and adderall 20mg when they intended for extended release because that'southward a dosage form change. Please don't leave someone hanging on a Fri later on DDS wrote for a dozen 5/300 and only chosen yous to say it'due south okay to get 325. Only question there is why it seems dentists are only ones that write for 5/300. It'southward too probably not a good thought to change fentanyl to dilaudid. Only time I could envision that would be hospice, aside from some bizarre oncology situation, and they tin take the time to set up it by faxing or Eastward-Rx'ing a new Rx.
